*This application form is common for both commercial and recreational pleasure vessel, so the applicant needs mark with an ‘X’ on whichever option he/she wants to make.

*Likewise, the applicant should mark the check box as applicable to renewal of registration at the top of the application.

*Places that are not applicable to the type of vessel that you own should be filled with an N/A.

*Application form can be typed or completed neatly by hand as applicable to the applicant’s requirement.

*It is important that the completed form must contain all the information required properly and the form must be filled legibly, in order to get accepted by the Authority.

*In case the previously registered certificate is unavailable, the applicant have to make a statement in writing about the absence of that certificate.
