#To apply for Child Immunisation in person, the applicant has to visit the nearest clinic or health facility in his/her province.&lt;br /&gt;

#Contact the Department of health and enquire for a nearest health facility or a clinic in your province.&lt;br /&gt;
#Visit the clinic and obtain the application form to apply for Child Immunisation, from the concerned staff or counter.&lt;br /&gt;
#Make sure you have attached all the documents listed in the ”Required Documents” section of this page.&lt;br /&gt;
#Then, complete the application form and submit it along with all required documents to the concerned staff or nurse (as directed).&lt;br /&gt;
