For service by mail, fax or email to the Slovak Registry offices. The Birth Certificate Application must be signed and dated in front of a notary public. Payment is accepted by credit card (MasterCard, VISA, or Discover), check or money order and is deposited upon receipt.

Birth certificates can be obtained by;
*The person named on the birth certificate.
*A parent of the person named on the birth certificate (requesting parent's name must be on birth certificate).
*Only by order of a Slovak Court may a spouse, child or other persons obtain a copy of a birth certificate.

The registration of births and issuing of birth certificates in the Slovak Republic is regulated by the Act No. 154/1994 Coll. on Civil Registers.

Local People's Committees in many cities, including Horna Suca, District Trencin are authorized to issue birth certificates, marriage certificates, and similar civil registration documents in Bratislava, Piestany, Kosice,or in other cities.

* Name, surname, maiden name, day, month, year and place of birth, birth identity number, citizenship and permanent residence of parents of the child,
