==Procedure==
You are a Slovakia citizen. You can apply for Citizenship Certificate on a suitable District Office, diplomatic mission or consular office of the Slovak Republic. The Citizenship Certificate is issued by the District Office at written request for a citizenship certification.To do it, please follow:
#Prepare the required documents:
#*A completed written request or application for a citizenship certification. This application should include this information:
#**Name, surname, maiden name, birth number (if have), date and place of birth
#**Your residence address and address of your last residence in the Slovak Republic
#**Date and signature
#*Your proof of identity as valid ID card, passport, etc.
#*Your Birth certificate
#*Your proof of personal status, which is a marriage certificate, a final sentence or a certificate of divorce, death certificate of spouse, if you are a married man or a woman, divorced or widowed.
#*Other documents necessary to decide about you, if asked by the District Office.
#Visit a suitable District Office, diplomatic mission or consular office of the Slovak Republic

#*Present the required documents and follow all of officer’s guiding to apply for Citizenship Certificate and pay the applicable fee.
#*The Certificate is issued on a form issued by the Ministry, usually within 30 days of the requestdepending on where you requested it a District Office, a diplomatic mission or consular office.
