==Eligibility==
*The applicant should have lived in Ontario for 12 months. 
*The applicant must remain in Ontario until the change of name document has been approved. 
*The applicant must be 18 years of age or older. 
*If the applicant is under 18 years, they can apply under certain conditions, and these are: 
**If the applicant is changing the name of a minor but the minor turns 18 before the application is processed, in which case, a new application will need to be submitted. 
**The applicant needs to make sure there is enough time to process the application before submitting an application to a registry agent. 
 
==Fees==
*The applicant will have to pay a fee of 137$ for an adult change of name.
*For changing the name of a child, the applicant will either have to pay 122$ or pay 22$ if the form is being submitted along with the parent’s change of name form. The same applies to a single name change.
*If the applicant is reclaiming their indigenous name, they can do so free of cost.
