===Apply In-Person===&lt;br /&gt;
#An individual Mayor’s Permit is required for workers or employees (temporary or permanent) who are working within the jurisdiction of the City. This said permit is called Occupational Permit. Here&amp;#039;s how you can apply for one. First you will need to obtain a community tax certificate. You need a valid community tax certificate number (for the current year) to complete the application form. &lt;br /&gt;
#Next, you will need to go to clinic and have a chest X-Ray, urinalysis and fecalysis. &lt;br /&gt;
#Complete the [https://www.muntinlupacity.gov.ph/wp-content/uploads/2017/04/occupational.pdf Occupational Permit Application Form]. A link is provided under &amp;#039;Documents to Use&amp;#039; section which you can click to download the form. Do not forget to sign the form and attach a 1x1 picture of you.&lt;br /&gt;
#Submit the completed form and all the lab results to the Business Permit and Licensing Office of the Muntinlupa City Government.&lt;br /&gt;
#Pay the application fee.&lt;br /&gt;
#The BPLO officer will inform you when you can pick up your occupation permit.&lt;br /&gt;
