#Visit the office and obtain the application forms to be completed for renewal of Private Practice License, from the relevant department.
#Complete the forms with appropriate information under relevant sections. Attest the forms with the applicant signature once completed.
#The interested applicants, who wish to be registered and certified, must submit an application Form for accreditation, as well as all the relevant documentation to the office.
#Once completed filling the application form, make sure that the applicant attached all the documents listed in the “Required Documents” section of this page.
#Payment of appropriate fees should be made and that can be found under the "Fees" section of this page.
#Then, submit it in person to the relevant authority at the office. Make the stipulated payment as directed by the officials.
#The office will forward the applications to the Accreditation and certification Committee for processing.
#The Accreditation and Certification Committee will review and use the set criteria for assessment of applications.
#The results of the applicants to provide certificate will be sent to the Ghana MOH office for final approval.
#Collect his/her renewal License from where the applicant application was submitted within 14 days from the date the application was received at Ghana MOH office in case there is no rejection.
