You may order a copy of a birth, death or fetal death record through your local County Health Department. Counties have computer access from 1917 to present for birth records for the entire state as well as births that occurred in their county.

The Florida Department of Health, Office of Vital Statistics (Florida Vital Records) issues certified copies of Florida death certificates for events which occurred in the State of Florida.

By law, death certificates with "Cause of Death" information can only be issued to the following:
#Parent
#Child (if of legal age)
#Grandchild
#Any person who provides documentation that he or she is acting on the behalf of any of the before named persons or by court order directing Vital Statistics whom to issue a death record too.
#If you are requesting a Death Certificate with "Cause of Death", you must explain your relationship to the person whose record you are requesting. If a relationship is not stated, you will be issued a Certified copy excluding cause of death information.
