When seeking a passport for a minor under age 16, in addition to document required both parents must come with the child and their own passports or other government-issued identification with the bearers name, photo and signature. If both parents cannot come, the absent parent must provide a notarized written authorization form (Statement of Consent or Special Circumstances: Issuance of a Passport to a Minor Under Age 16) for the issuance of the passport and include the absent parents passport or a notarized copy of the absent parents passport/identification.<br />
